Ashy Academy 2026/27 Player Trials – Registration Information and registration

Terms and conditions

• By submitting this registration form, I acknowledge and agree to the following:
  
  1. I am registering the player to participate in the Ashy Academy 2026/27 trial and assessment process, delivered by S4S Academy in partnership with Ashburton United Soccer Club.
  
  2. I understand that submitting this registration form does not guarantee the player a place within an Ashy Academy team for the 2026/27 season.
  
  3. I understand that all team selections and placement decisions will be determined by the Ashy Academy coaching and technical staff.
  
  4. Players may be assessed during trial sessions, regular training sessions, matches and other football activities where applicable.
  
  5. Selection decisions may consider the player’s technical ability, tactical understanding, physical development, attitude, behaviour, commitment, attendance, coachability and suitability for the relevant team.
  
  6. I agree to ensure that the player attends all allocated trial and assessment sessions wherever reasonably possible. Any absence must be communicated to S4S Academy as early as possible.
  
  7. I understand that trial dates, session times and venues may change due to weather, ground availability, council directions or other circumstances outside the control of S4S Academy and Ashburton United Soccer Club.
  
  8. I confirm that the player is physically able to participate in football activities and that all relevant medical conditions, allergies, injuries and additional support requirements have been disclosed.
  
  9. I authorise S4S Academy and Ashburton United Soccer Club staff to provide first aid or obtain emergency medical assistance for the player where reasonably necessary.
  
  10. I acknowledge that football is a physical activity and that participation may involve a risk of injury.
  
  11. I agree that the player and their family will behave respectfully towards coaches, staff, volunteers, teammates, opponents, referees and other families throughout the trial and assessment process.
  
  12. I understand that inappropriate, aggressive or disrespectful behaviour from a player, parent or guardian may result in the player being removed from the trial and assessment process.
  
  13. I consent to receiving communications from S4S Academy and Ashburton United Soccer Club regarding trials, assessments, team selections and relevant Academy program information.
  
  14. I understand that the personal information provided through this form will be used for trial administration, player assessment, team planning, safety and communication purposes.
  
  15. I confirm that all information provided in this registration form is accurate and complete to the best of my knowledge.
  
  16. I confirm that I am the player’s parent or legal guardian, or that I have the authority to complete this registration on their behalf.
